Salary - £23,800

6 months ftc  - Possibility of extension                                                                                     

5 days per week , Monday-Sunday 9am-5pm - 37.5 hours per week

We are looking for a Patient administrator to join our team where you will closely collaborate with hospital discharge teams and patients providing a high quality service from the outset and throughout 

This position offers a unique opportunity to work towards quick and safe discharge plans, supporting families during challenging times.

Every day you will…

  • Coordinate both short-term and long-term care home packages for clients, ensuring a swift, safe, and accurate discharge plan for patients
  • Liaise with care homes, care agencies, senior health and social care staff, patients and their families handed conversations in a delicate, sympathetic manner.
  • Facilitate and relay updates, serving as the liaison between discharge teams, local authorities, clients, and care providers to ensure the successful delivery of high-quality services and timely patient discharges

To thrive in this role you must ideally have...

  • Previous customer service experience with excellent written and verbal communication skills
  • Previous administration experience with good attention to detail
  • The ability to organise and prioritise workload effectively
